Physician Assistant or Nurse Practitioner — Post-Acute Care — $150/hour

BridgeCare Medicine is a physician-led medical group hiring physician assistants and nurse practitioners to care for adult residents in skilled nursing and post-acute facilities. Full-time and flexible part-time opportunities are available, with schedules arranged around your availability and facility needs.

Locations and travel

This is an on-the-road position with in-person assignments in Bennington, Rutland, Springfield, and/or St. Johnsbury, Vermont. Tell us which towns you can reliably cover. Facility assignments and schedules will be agreed before clinical work begins.

What you’ll do

• Evaluate new symptoms and manage acute and chronic conditions within your scope of practice.

• Review medications, hospital records, and laboratory results, and follow through on needed care.

• Coordinate with nursing staff and the covering physician, escalating changes in condition when appropriate.

• Complete clear, timely documentation and follow-up.

What you’ll need

• Graduation from an accredited PA or NP program.

• Appropriate national certification.

• Vermont licensure appropriate to your role before starting clinical work.
